First Visit

Thank you for choosing our office for all your medical care needs. To assist us with your care, please arrive at least 30 minutes early to complete the registration process. Please bring the following items to your first visit:

Your insurance card and a photo ID

A list of medications with dosage that you are currently taking - or your medication bottles

Results of any related medical or laboratory tests

Completed New Patient Packet (Click here) or complete it on the portal link included in the webiste

Your Co-pay and Deductible not met are required at the time of your appointment

Proof of premium payment for Exchange products will be requested at time of appointments
