Insurance
It is our mission to provide you with affordable, high-quality treatment that fits your budget! We will bill most insurance carriers for covered services as long as proof of insurance is provided at the time of service. Co-Payments are expected at the time of services. We offer a variety of payment options to meet your needs. Our office accepts payments by cash, checks, and Visa, MasterCard and Discover credit cards.
Please visit Financial and Insurance for additional information.

If your insurance plan requires a referral to see the specialist, please contact your primary care physician. If a referral is not received at the time of your appointment, you may be asked to reschedule.

Financial and Insurance
Our office is committed to helping you maximize your insurance benefits.
Because insurance policies vary, we can only estimate your coverage in good faith and cannot guarantee coverage due to the complexities of insurance contracts.
Payment of co-pays, deductibles and patient balances are due when services are rendered. Patients who have insurance through the Health Exchange need to show proof of insurance premium payment at time of service.

Insurance Policy
On your first visit to our practice, bring your current insurance card with you. If you change insurance companies or join another plan, please be sure to notify our insurance department of this change immediately.
It is the patient’s responsibility to verify with their carrier that their physician is a participating provider.
As a courtesy to our patients, we will submit insurance claims for you. We will fully attempt to help you receive full insurance benefits; however, you are personally responsible for your account, and we encourage you to contact us if your policy has not paid within 30 days. Prior to making an appointment always verify we participate with your plan as our participation with certain plans can change. With the multitude of available health plans, patient involvement is paramount.
